ANNEX 1 - MANDATORY CONDITIONS
No supply of alcohol may be made under this licence
a. at a time when there is no designated premises supervisor in respect of it or,
b. at a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended
Every retail sale or supply of alcohol made under this licence must be made or authorised by a person who holds a personal licence.
ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
There shall be a Personal Licence holder present at all times the premises is open to the Public, together with sufficient staff to assist him/her in meeting the licensing objectives.
The Premises Licence Holder shall comply with all relevant legislation including fire safety and health and safety.
The sale of alcohol to any customer who is seen to be intoxicated or boisterous shall be refused.
Service shall be refused to known trouble makers.
The toilets shall be maintained in good and hygienic order by regular inspection while open to the public.
The sale of alcohol shall be refused to anyone who appears to be under 18 by requiring evidence of identity and displaying appropriate posters.
ANNEX 3 - CONDITIONS ATTACHED AFTER A HEARING BY THE LICENSING AUTHORITY
Intoxicating liquor shall not be sold or supplied on the premises otherwise than to persons taking table meals there and for consumption by such a person as ancillary to his meal.
The supply of intoxicating liquor shall be by waiter/waitress service only and to persons seated at tables.
The licence holder and or DPS is to support and rigorously enforce the Challenge 21 Proof of Age policy. Any person who looks or appears to be under the age of 21 shall be asked to provide identification that they are over the age of 18. The following are the only forms of identification acceptable: photo driving licence; passport; Proof of Age Standards Scheme Card; any other locally or nationally approved form of identification which may be introduced in the future.
All staff to have received suitable training in relation to the proof of age scheme to be applied upon the premises. Records to evidence this will be made available to an authorised officer upon request.
Another member of staff shall be nominated to act for the Designated Premises Supervisor, in their absence, whose identity is known by all other staff when such absence occurs.
