ANNEX 1 - MANDATORY CONDITIONS
1.1 No supply of alcohol may be made under this licence
a. At a time when there is no designated premises supervisor in respect of it or,
b. At a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended
1.2 Every retail sale or supply of alcohol made under this licence must be made or by a person who holds a personal licence.
1.3 The premises licence holder or club premises certificate holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol.
The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.
ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
2.1 Appropriate measures shall be taken to deter thieves and shoplifters
2.2 Appropriate measures shall be taken to ensure the safety of those members of the public who visit the store.
2.3 Appropriate measures shall be taken to ensure the prevention of public nuisance.
2.4 Appropriate measures shall be taken to ensure the protection of children from harm.
ANNEX 3 - CONDITIONS ATTACHED AFTER A HEARING BY THE LICENSING AUTHORITY
3.1 CCTV shall be installed internally at the premises and shall comply with the following:
a. The CCTV system shall be installed, maintained and operated to the reasonable satisfaction of Lancashire Constabulary. The entrance and the alcohol display area are to be covered by the system. The system will incorporate a camera covering the entrance doors and will be capable of providing an image which is regarded as identification standard.
b. The system shall display on any recording the correct time and date of the recording.
c. The system shall make recordings during all hours the premises are open to the public.
d. VCR tapes or digital recording shall be held for a minimum of 31 days and 21 days respectively, after the recording is made and will be made available to the Police or any authorised persons acting for Responsible Authority for inspection upon request.
e. The system shall, as minimum, record images of the head and shoulders of all persons entering the premises.
3.2 A staff member who is conversant with the operation of the CCTV system shall be on the premises at all times that the premises is open to the public. This staff member will be able to show police recent data or footage with the absolute minimum of delay when requested. This data or footage playback should be almost instantaneous, with subsequent copies of footage to be made available within 7 days if requested.
3.3 An incident report will be completed, in which shall be recorded (as far as reasonably practicable) incidents of crime and disorder, refused admission or ejection and occasions when Police are called to the Premises.
Such reports shall be available for inspection by a Police Officer or authorised officer of a Responsible Authority.
3.4 At least one Personal Licence holder will be available or a signed Delegated Authority sheet left in store at all times.
3.5 Refresher training will take place at 3 monthly intervals. The training will be documented and made available to the authorities on request.
3.6 Refusals forms shall be completed on tills detailing all refused sales of under-age products to suspected underage customers
3.7 There will be a "Challenge 25" scheme in operation at the premises, requiring any individual who appears to be under the age of 25 to provide identification to prove that they are over the age of eighteen. In the event that the individual is unable to provide said identification, then the sale of alcohol will be refused. The only forms of identification which we be accepted as proof of age are:
- a valid UK Passport;
- a valid UK photocard driving licence;
- A PASS card; and / or,
- any other nationally or locally approved form of identification which may be introduced in the future
