ANNEX 1 - MANDATORY CONDITIONS
No supply of alcohol may be made under this licence
a. At a time when there is no designated premises supervisor in respect of it or,
b. At a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended
Every retail sale or supply of alcohol made under this licence must be made or authorised by a person who holds a personal licence.
ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
All staff shall have a personal record of training and checklists shall be provided for key policies and procedures (Challenge 25, drunkenness on premises etc) with supervision during their initial employment period. All training shall be logged and records kept for information.
An authorisation of sales, signed and dated by the DPS, shall be kept at the premises showing all persons authorised by them to make sales of alcohol at the premises. All staff involved in sales shall be at least 18 years of age and will receive training to promote the licensing objectives. All training to be documented and records made available on request to a police officer or an authorised officer acting for a Responsible Authority.
An alarm system shall operate when the premises are closed.
The Premises Licence Holder or a nominated person shall ensure the responsible promotion of alcoholic drinks with the Best Practice Guidelines from the Portman Group Code of Practice.
The premises shall sell food and promote sensible consumption practices and clearly label lower alcohol and alcohol free products.
The licence holder is to support and rigorously enforce the Challenge 25 Proof of Age policy. Any person who looks or appears to be under the age of 25 shall be asked to provide identification that they are over the age of 18. The following are the only forms of identification acceptable: photo driving licence; passport; Proof of Age Standards Scheme Card, or any other nationally or locally approved form of identification which may be introduced in the future. If no suitable identification is provided, sale of alcohol shall be refused.
An incident book shall be maintained, in which shall be recorded all refused sales to suspected under age / drunken persons. The book shall be available for inspection by a police officer or an authorised officer acting for a Responsible Authority.
All staff shall have proof of age training and at the discretion of the Premises Licence Holder or a nominated person, permanent staff may be asked to undertake formal training leading to Personal Licence holder status.
All sales of alcohol shall be made face to face to customers in the shop. No alcohol may be sold via internet/phone sales and delivered
The premises shall to be equipped with a closed circuit television system, which is appropriate to its purpose and provided and maintained to the satisfaction of the enforcement authorities.
