Annex 1 - Mandatory Conditions
No supply of alcohol may be made under the premises licence:-
(a) at a time when there is no designated premises supervisor in respect of the premises
licence,
or
(b) at a time when the designated premises supervisor does not hold a personal licence or
his personal licence is suspended.
Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
Annex 2 - Conditions Consistent with the Operating Schedule
Staff shall be given appropriate training in respect of the provisions of the Licensing Act 2003 and the licensing objectives.
There will be in place for the premises a policy to prevent the sale or supply of alcohol to persons under 18 years of age. That policy will require any person who appears to be under the age of 21 years to produce a recognised proof of age card accredited under the Proof of Age Standards Scheme (PASS) or if a proof of age card is not available a photo driving licence or passport indicating that they are over 18 years of age.
The Designated Premises Supervisor shall attend the premises on a regular basis.
The number of persons allowed on the premises shall be restricted to 56 indoors and 18 outdoors.
All fire exits shall be clearly lit and marked and shall be kept clear and unobstructed at all times.
Appropriate fire fighting equipment and fire precautions shall be maintained at the premises. Such equipment shall be regularly tested.
The following safety certification shall be available:
) A current Electrical Inspection Report Certificate.
) A current Fire Alarm Test Report.
) A current Emergency Lighting and Test Report.
The disposal of rubbish shall be in accordance with any guidelines prescribed by Fylde Borough Council.
All stock deliveries shall take place at a reasonable time.
A licensed taxi service shall be promoted by the premises and staff, when required, shall organise
taxis to transport customers home.
Children shall be required to be supervised by the accompanying adult at all times.
Annex 3 - Conditions attached after a hearing by the Licensing Authority
All staff shall receive suitable training in relation to the proof of age scheme to be applied upon the premises. Records to evidence this shall be made available to an authorised officer upon request.
