ANNEX 1 - MANDATORY CONDITIONS
No supply of alcohol may be made under this licence
a. at a time when there is no designated premises supervisor in respect of it or,
b. at a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended
Every retail sale or supply of alcohol made under this licence must be made or authorised by a person who holds a personal licence.
ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
The premises are to be equipped with a closed circuit television system, and the tapes shall be retained for a minimum period of 31 days.
There will be in place for the premises a Challenge 21 policy to prevent the sale or supply of alcohol to persons under 18 years of age. That policy will require any person who appears to be under the age of 21 years to produce a recognised proof of age card accredited under the Proof of Age Standards Scheme (PASS) or if a proof of age card is not available a Citizen Card or passport indicating that they are over 18 years of age.
Staff shall be given appropriate training in respect of licensing laws, the operation of a “Refusals Book” and refresher training on a regular basis.
The premises shall operate within any required litter/refuse regulations adopted by Fylde Borough Council.
There shall be prominent signage throughout the store confirming the legal minimum age for the purchas of alcohol.
