ANNEX 1 - MANDATORY CONDITIONS
No supply of alcohol may be made under this licence
a. at a time when there is no designated premises supervisor in respect of it or,
b. at a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended
Every retail sale or supply of alcohol made under this licence must be made or authorised by a person who holds a personal licence.
ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
All staff shall be made aware and trained of Health and Safety objectives.
Staff shall be trained in relation to the use of “Challenge 21” proof of age policy.
Hot drinks, soft drinks and non-alcoholic drinks shall be available as an alternative to alcohol at all times.
The provision of food, including full meals and snacks, shall form a substantial element of the operation of the premises.
The premises shall be operated in accordance with the principles outlined by the Nightsafe Initiative as related to the premises.
A security system, reinforced glass and CCTV shall be installed.
Glasses shall be progressively replaced with those of toughened glass.
Whenever the premises are open after midnight, patrons shall be reminded to leave the premises without causing annoyance or disturbance to local residents.
The premises shall benefit from a Health and Safety Policy, Public Liability Insurance, Fire Alarm, Emergency Lighting, Fire Extinguishers and Safety and Warning signs.
Suitable and sufficient disabled facilities and access shall be provided.
Adequate first aid provision shall be available at all times.
Glasses and bottles shall be removed on a regular basis.
A notice shall be displayed to prevent the sale or supply of alcohol to persons under 18 years of age.
The Designated Premises Supervisor, when present, and all members of staff shall ensure that all lawful instructions and/or directions given by the Police are complied with.
There shall be no drinks promotions that allow unlimited supply of alcohol for a set price.
All staff shall be made aware of safety matters in relation to children.
ANNEX 3 - CONDITIONS ATTACHED AFTER A HEARING BY THE LICENSING AUTHORITY
The premises shall be equipped with a closed circuit television system which meets the criteria detailed in the Lancashire Constabulary Guidance Document CCTV/AW, and which is well maintained and fully functional during the hours the premises are open to the members of the public.
A staff member who is conversant with the operation of the CCTV system shall be on the premises at all times the restaurant is open to the public. This staff member shall be able to show police recent data or footage with the absolute minimum of delay when requested. This data or footage reproduction should be almost instantaneous.
There shall be substantial refreshment available up until 30 minutes before the terminal hour of the supply of alcohol.
All doors and windows shall remain closed during the performances of regulated entertainment except for entrance and exit purposes in order to reduce the noise impact to residential properties.
Patrons shall be returned indoors after 2300 to reduce the noise impact on residential properties.
Patrons shall not be permitted to take drinks outside when smoking after 2300.
The music piped outside shall be curtailed after 2200 to reduce noise impact on neighbouring dwellings.
Dancing shall not include any nudity or semi-nudity (such as striptease or lap dancing, pole dancing) or any similar adult entertainment).
