ANNEX 1 - MANDATORY CONDITIONS
1.1 No supply of alcohol may be made under this licence
a. At a time when there is no designated premises supervisor in respect of it or,
b. At a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended
1.2 Every retail sale or supply of alcohol made under this licence must be made or by a person who holds a personal licence.
1.3 The premises licence holder or club premises certificate holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol.
The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.
ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
2.1 A CCTV system shall be installed, or the existing system maintained, and such system shall be fit for the purpose.
2.2 The system shall incorporate a camera covering each of the entrance doors and the alcohol display areas and shall be capable or providing an image which is regarded as identification standard. The precise positions of the cameras may be agreed, subject to compliance with Data Protection legislation, with the police from time to time.
2.3 The system shall incorporate a recording facility and any recording shall be retained and stored in a suitable and secure manner for a minimum of 31 days and shall be made available, subject to compliance with Data Protection legislation , to the Police for inspection on request. If the premises are using a video recording system, the cassette tape shall be used on no more than 12 occasions.
2.4 The system shall display on any recording the correct time and date of the recording.
2.5 A system shall be in place to maintain the quality of the recorded image and a complete audit trail maintained.
2.6 Notices informing customers of the operation of the system shall be prominently displayed,
2.7 The system shall be maintained and be fully operational throughout the hours that the premises are open for any licensable activity.
2.8 The Premises Licence Holder shall at all times maintain adequate levels of staff. Such staff levels shall be disclosed, on request, to the Licensing Authority and Police.
2.9 Signage shall be displayed at the exit of the premises requesting customers leaving the premises late at night to do so quietly and with consideration as not to disturb nearby residents.
2.10 All cashiers shall be trained to require evidence from any person seeking to buy alcohol and appearing to the cashier to be under the age of 18. The evidence shall be photographic, such as passport or photographic driving licence until other effective identification technology (for example thumb print or pupil recognition) is introduced. Prominent notices shall be displayed at points of sale advising customers that they may be asked to provide evidence of age.

This plan indicates the proposed position for the display and storage of alcohol. Alcohol may be displayed or stored, from time to time, in different positions within the premises.
