ANNEX 1 - MANDATORY CONDITIONS
No supply of alcohol may be made under this licence
a. At a time when there is no designated premises supervisor in respect of it or,
b. At a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended
Every retail sale or supply of alcohol made under this licence must be made or authorised by a person who holds a personal licence.
The premises licence holder or club premises certificate holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol.
The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.
ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
The premises licence holder shall endeavour to maintain a good relationship with the relevant authorities in order to assist with the prevention of crime and the maintenance of public safety.
The premises licence holder shall take all necessary measures to prevent noise, light, air and other forms of pollution.
Staff shall be trained to deal with crime and disorder and the relevant authorities shall be informed when crime or illegal activity could potentially occur.
The premises shall be properly secured and well lit.
The premises shall encourage initiatives such as the Naming, Packaging and Promotion of Alcoholic Drinks programme.
An emergency evacuation procedure shall be put in place to ensure the safety of customers in the event of a fire or other emergency. Staff shall receive training to deal with emergency situations safely.
The Premises Licence Holder shall, as far as reasonably practical to do so, ensure that the premises are kept free from litter and shall endeavour to dispose of waste safely and in accordance with any environmental legislation.
The Premises Licence Holder shall keep noise to a minimum level and encourage customers to respect the rights of nearby neighbours when leaving the premises.
Waste shall be disposed of quickly and safely so that it does not become an environmental nuisance.
Staff shall be trained to recognise the signs of persons buying alcohol for persons under the age of 18.
ANNEX 3 - CONDITIONS ATTACHED AFTER A HEARING BY THE LICENSING AUTHORITY
An authorisation of sales, signed and dated by the DPS, shall be kept at the premises showing all persons authorised by them to make sales of alcohol at the premises. All staff involved in sales will be at least 18 years of age and will receive training to promote the licensing objectives. All training to be documented and records made available on request to a Police Officer or an authorised officer acting for a Responsible Authority.
CCTV shall be installed internally and externally at the premises and will comply with the following
i) The CCTV system shall be installed, maintained and operated to the reasonable satisfaction of Lancashire Constabulary. All public areas of the premises are to be covered by the system.
ii) The system will display on any recording the correct time and date of the recording.
iii) The system will make recording s during all hours the premises are open to the public.
iv) VCR tapes or digital recordings shall be held for a minimum of 31 days after the recording is made and will be made available to the Police or any authorised officers acting for a Responsible Authority for inspection upon request.
The licence holder is to support and rigorously enforce the Challenge 21 Proof of Age policy. Any person who looks or appears to be under the age of 21 shall be asked to provide identification that they are over the age of 18. The following are the only forms of identification acceptable: UK photo driving licence; passport; Proof of Age Standards Scheme Card; or any other locally or nationally approved form of identification which may be introduced in the future. If no suitable identification is provided, sale of alcohol must be refused.
Signs promoting the Challenge 21 policy will be predominantly displayed on the premises.
All staff to have received suitable training in relation to the proof of age scheme to be applied upon the premises. Records to evidence this shall be made available to an authorised officer upon request.
An incident book will be maintained in which shall be recorded all refused sales to suspected underage/drunken persons. The book shall be available for inspection by a police officer or an authorised officer acting for a Responsible Authority.
