ANNEX 1 - MANDATORY CONDITIONS
No supply of alcohol may be made under this licence
a. At a time when there is no designated premises supervisor in respect of it or,
b. At a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended
Every retail sale or supply of alcohol made under this licence must be made or authorised by a person who holds a personal licence.
The premises licence holder or club premises certificate holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol.
The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.
ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
All staff shall receive suitable training in relation to the proof of age scheme to be applied upon the premises. Records to evidence this shall be made available to an authorised officer upon request.
There shall be in place appropriate policies and training programmes, subject to regular review, in order that the Licensing Objectives may be achieved.
A till prompt shall appear on the initial sale of alcohol that shall remind the seller of the Think 25 policy and remind staff not to sell alcohol to anyone under the age of 18.
The premises shall display signage around the premises informing both customers and staff of the Think 25 policy.
The premises are to be equipped with a closed circuit television system, which is appropriate to its purpose and provided and maintained to the satisfaction of the enforcement authorities.
There shall be maintained, on the premises, at all times, an adequate and appropriate supply of first aid equipment and materials.
The premises shall adhere to all rules and regulations relating to public safety.
Adequate and appropriate means of fire fighting equipment shall be available on the premises.
A clear, legible and conspicuous notice requesting patrons to avoid causing noise, nuisance or disturbance to local residents shall be displayed at every exit.
There shall be in place for the premises a policy to prevent the sale or supply of alcohol to persons under 18 years of age. That policy shall require any person who appears to be under the age of 25 years to produce a recognised proof of age card accredited under the Proof of Age Standards Scheme (PASS) or if a proof of age card is not available a photo driving licence or passport indicating that they are over 18 years of age.
Point of sale notices shall be displayed advising that any person who appears under the age of 25 shall be asked for proof of age.
Training shall be given to all staff of the consequences for adults who buy alcohol on behalf of children.
All refusals shall be recorded in a refusals book kept in store and monitored by the store manager.
A notice or notices shall be displayed in and at the entrance to the premises where they can be clearly seen and read and shall indicate that it is unlawful for persons under 18 to purchase alcohol or for any person to purchase alcohol on behalf of a person under 18 years of age.
