ANNEX 1 - MANDATORY CONDITIONS
No supply of alcohol may be made under this licence
a. At a time when there is no designated premises supervisor in respect of it or,
b. At a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended
Every retail sale or supply of alcohol made under this licence must be made or authorised by a person who holds a personal licence.
The premises licence holder or club premises certificate holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol.
The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.
ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
All staff shall be trained and aware of Licensing legislation, particularly in relation to the prevention of underage sales. They shall be required to sign a register confirming that they have undertaken training and are aware of their responsibilities. Training shall be repeated at frequent intervals, at least bi-annually.
Fire safety equipment shall be provided at the premises to comply with the relevant recognised standards, and staff shall be trained on the use of this equipment.
If anyone attempting to purchase alcohol appears to be under 21 the on duty manager shall be called. The manager shall only accept photographic ID as proof of age (passport, photo driving licence or PASS card). If no ID is provided no sale takes place.
ANNEX 3 - CONDITIONS ATTACHED AFTER A HEARING BY THE LICENSING AUTHORITY
Recordable CCTV to a high standard and format as acceptable for evidential use if required shall be installed at the premises. This system will be capable of recording the areas of the premises susceptible to alcohol related crime together with the entrance and exit of the premises. All imagery will be kept for as long as the system is able (whilst ensuring the high quality of the images are retained) but, in any event for a minimum of 28 days. Copies will be made available as soon as reasonably practical and in any event, within 48 hours.
An authorisation of sales, signed and dated by the DPS, shall be kept at the premises showing all persons authorised by them to make sales of alcohol at the premises.
All staff involved in sales will receive training to promote the licensing objectives. All training will be documented and records made available on request to a police officer or an authorised officer acting for a Responsible Authority.
An incident book shall be maintained, in which shall be recorded all refused sales of alcohol to suspected under age/drunken persons. The book shall be available for inspection by a police officer or an authorised officer acting for a Responsible Authority.
No entertainment of an adult nature will take place on the premises.
